Des Moines County Public Health will be offering a COVID-19 vaccination clinic for students at Burlington High School on April 28, 2021. BHS students who are 16 years old on or before April 28 are eligible to receive the vaccine at this clinic. This is a two dose vaccine and the 2nd dose will be given at BHS on May 19. Parents/Guardians (or students who are 18 or older) MUST complete and sign this two page form and return them to the Burlington High School Office by April 23 to be scheduled as part of this clinic.
